Year 4 Selectives

1. Click here to load the Application form for a Basic Science and Medical Humanities Selective.
2. Once the form is loaded, you can type your form answers directly onto the form within the web browser window.
3. Print a copy of the form for your records.
4. Click the disk icon in the upper left corner of the browser window to save a copy of the pdf file to your local computer.
5. Send and email to Cindi Wolf at cawolf@utmb.edu and attach the saved pdf file.
